• New Patient Forms

  If your child is a new patients, that already has a prescription from their physician and a scheduled appointment for either an initial evaluation or ongoing individual therapy, please print out and complete all the forms provided. You must bring the completed forms to the scheduled appointment. If you are unable to open or print any page

  Please contact us at (602) 476-7519 and we will send them via mail or fax. If you do not bring these forms completed please allow, at least, an additional 20 minutes prior to your scheduled appointment time.

• Please print an express registration form and questionaire that is appropriate for your childs age. Fill them out completely, if you are unsure make as best a guess as possible but mark the question to review with the therapist. Please bring all forms with you to your scheduled screening. If you wish to speed up the process you can fax the information to (602) 445-4971 attention Screening.

  This is a fax direct to email account so your information is only seen by therapy and intake staff. If you cannot fill these forms out ahead of time you will be required to do so at the screening and should allow extra time to do so prior to your scheduled appointment. If you have been unable to print them for any reason please call (602) 476-7519 and copies can be sent via mail or fax to you.
